We have paved the way for excellence in Integrated Care and in July 2016 took the  pioneering step to form one of the first Integrated Care Organisations.  We now provide adult social care services alongside our established healthcare services, which has given us the chance to develop better, more joined up ways of working that deliver improved outcomes for the local community. 


Salford Together is at the forefront of developing neighbourhood and City-wide services in collaboration with the public through Salford’s Big Health and Care Conversation. We are now ready to recruit to the first key roles in new teams who will play a crucial role in taking this forward – with further opportunities to come.

Salford Together is a partnership across Primary, Acute and Mental Health, Social Care and Commissioning, which has been working to improve people’s experience of care, health and wellbeing whilst managing resources efficiently to support a financially stable health and care system.


You will be part of these new and exciting community based initiatives that are transforming our approach to health and care delivery in Salford. This is a unique opportunity for you to join one of our emerging services which includes Neighbourhood Enhanced Care Teams.

Our Enhanced Care Team will be a multi-disciplinary neighbourhood based team, made up of physical and mental health – and social care expertise.
The service aims to support people at risk of deteriorating health and wellbeing, to develop their own goals – working intensively with them to remain healthy and well for longer.

You will be an experienced and autonomous Consultant Practitionerwho can demonstrate advanced expertise in the assessment, treatment and management of a diverse and undifferentiated caseload of adult patients.

You will be an excellent communicator, adaptable, committed, well organised and work well under pressure. You must be able to travel around various community locations delivering care.

Secondments will be considered.
