Heywood, Middleton & Rochdale (HMR) within Community Services outreaching into Nursing and Care Homes.



An exciting opportunity has arisen for a number of Band 8a Advanced Practitioners to work within a Community based workforce providing specialist support across the borough of Heywood, Middleton and Rochdale.

Forming part of our Intermediate Tier of Services (ITS) the posts will be based within the Urgent Community Care team whose primary remit is to prevent unnecessary hospital admissions by supporting individuals to remain in their own homes by providing care at the point of need. You will assist individuals and their carers to proactively manage acute exacerbations of health need requiring urgent intervention, anticipate their future health needs, support self-care, develop appropriate plans, and work collaboratively with the wider multidisciplinary team including, partner organisations such as NWAS, GP/ primary care, pharmacy services and our developing Integrated Neighbourhood Teams. This exciting and challenging role offers the post holder the opportunity to provide high quality patient focused care in the community. The role supports and promotes the role of the Advanced Practitioner in the community through innovation, clinical skills and diagnosis, treatment, review and referral.

The post holder will be expected to engage with a range of service delivery within the Urgent Community Care Team e.g. Care home extra support, managing patients within Nursing and Care home settings; HEATT car and Virtual Beds

Successful applicants must have a broad experience in community and/or urgent care at a senior level. You must be motivated and enthusiastic and have proven experience of working as an autonomous practitioner.

Applicants must have the following essential qualifications:

  • MSc in Advanced Practice
  • Relevant professional health degree
  • Evidence of recent CPD
  • Current registration with NMC, HPC
  • NMP V300
